HUBUNGAN STATUS NUTRISI DENGAN KUALITAS HIDUP PASIEN CHRONIC KIDNEY DISEASE (CKD) YANG MENJALANI HEMODIALISIS DI RSUD ARIFIN ACHMAD PEKANBARU

Abstract


ABSTRACT

Chronic kidney disease (CKD) is a disease that mostly involves the urinary tract system. It is a public health problem with high prevalence, mortality, and health costs. Basically, the hemodialysis process carried out by CKD patients is able to increase serum creatinine, albumin, and pre-albumin. It is also able to normalize the protein catabolic rate and increase the food intake of CKD patients. However, most previous studies have found that the hemodialysis process can increase the prevalence rate of malnutrition and reduce the patient's quality of life. This research has the general aim of determining the relationship between nutritional status and quality of life in CKD patients undergoing hemodialysis. This type of research is observational-analytic with a quantitative approach and a cross-sectional design. The sampling technique used was purposive sampling, and the sample size was 65 people, with the criteria being that patients underwent regular HD at least twice a week. Based on the research results, it shows that the majority of respondents have poor nutritional status, namely 46 respondents (70.8%), and the majority of respondents have a good quality of life, namely 33 respondents (50.8%). Based on the results of bivariate analysis using chi-square, it was found that p value = 0.242 > α (0.05), which means there is no relationship between nutritional status and the quality of life of CKD patients undergoing HD. From the results of this research, it is hoped that Arifin Achmad Hospital can provide counseling to chronic kidney failure patients in the hemodialysis unit to be able to maintain nutritional status and improve the quality of life of CKD patients.

Chronic Kidney Disease (CKD) merupakan salah satu penyakit yang paling melibatkan sistem saluran kemih, yang menjadi masalah kesehatan masyarakat tingginya prevalensi, kematian, dan biaya kesehatan. Pada dasarnya proses hemodialisis yang dilakukan oleh pasien CKD mampu meningkatkan serum kreatinin, albumin, dan pre-albumin, kemudian juga mampu menormalkan laju katabolik protein serta meningkatkan asupan makanan pasien CKD Namun, sebagian besar penelitian sebelumnya menemukan bahwa proses hemodialisis dapat meningkatkan angka prevalensi malnutrisi dan menurunkan kualitas hidup pasien. Penelitian ini bertujuan umum untuk mengetahui hubungan status nutrisi dengan kualitas hidup pasien CKD yang menjalani hemodialisis. Jenis penelitian ini adalah analitik observasional dengan pendekatan kuantitatif dan desain cross sectional. Teknik pengambilan sampel menggunakan purposive sampling dan besar sampel adalah 65 orang dengan kriteria pasien yang menjalani HD regular minimal 2 kali seminggu. Berdasarkan hasil penelitian menunjukkan bahwa Mayoritas responden memiliki status gizi kurang baik yaitu sebanyak 46 responden (70,8%), Mayoritas responden memiliki kualitas hidup baik yaitu 33 responden (50,8%). Berdasarkan hasil Analisa bivariat menggunakan chi-square didapatkan bahwa p value = 0,242 > α (0,05), yang artinya tidak ada hubungan antara status nutrisi dengan kualitas hidup pasien CKD yang menjalani HD. Dari hasil penelitian ini diharapkan RSUD Arifin Achmad dapat memberikan konseling ke pasien gagal ginjal kronik di Unit Hemodialisis untuk dapat mempertahankan status nutrisi dan meningkatkan kualitas hidup pasien CKD.
